All incoming University students must provide certain health history information before arriving on campus. It is required that students have certain immunizations before coming to school.

Information For The New Students
All students are required to have a completed health details from his/her physician and file to be submitted with the admin before the start of classes.
Complete health details includes:
  1. A physical examination
  2. Present medical condition
  3. Past medical history
  4. Known medical condition
  5. Completed immunization record
  6. History of Food Allergy
  7. History of Drug allergy
  8. Family medical history
  9. On medications (if any)

Required Immunisation
  1. A booster of tetanus, diphtheria and pertussis (Tdap) within the last 10 years.
  2. Two MMR (measles, mumps and rubella) shots or blood tests indicating immunity to these conditions. The first shot must be given AFTER 12 months of age.
  3. A tuberculosis skin test depending on your country of origin and tuberculosis history.
